New Patient Appointment

If you have scheduled a consultation with Dr. Garcia and will be seen at Center for Women's Surgery for the first time, you will need to fill out the required forms.  For your convenience, we have provided these forms online.  Click here to download these forms.  

You will also need to bring your medical records or have them sent by mail or faxed to our office.  Click here to see which medical records are needed.  Please confirm with the office that all medical records and imaging studies (films or CD's) have been received by Center for Women's Surgery before your visit.

Arrival Time

We strive to respect all patients and their appointments with Center for Women's Surgery.  If you will be visiting us for the first time please allow for extra time before your appointment for completion of registration information and parking.  We ask that you allow an additional 45 minutes.  If your forms are filled out in advance, please allow approximately 30 minutes for parking and locating our office.  Patients who are more than 15 minutes late for the first appointment will be asked to reschedule.  If you need to reschedule your appointment please notify us at least 24 hours in advance so we may utilize the time for other patients.

Parking

Parking lots and garages are available to accommodate visitors to the Professional Building adjacent to Presbyterian Hospital.  However, parking spaces are often difficult to find.  This may cause a delay in arriving to the office on time for your appointment.  Please allow additional time prior to your appointment for parking.

Insurance

Please bring your insurance identification card to your visit.  If you insurance company requires an office co-payment, please be prepared to pay it in full when you arrive.  If your visit is not covered by a health plan or insurance, you will be asked to pay for your visit on arrival.  For your convenience, we accept cash, checks and credit cards. We will be happy to accommodate for languages other than English as well for Sign Language.  Please notify us at the time of your appointment if this special arrangement is needed.
