We are contracted with several health insurance plans.  If your plan is listed below, you qualify for "in-network" benefits for treatment received at our office.

If your insurance plan is not listed above, you may still be able to receive services at our office.  Many health insurance plans provide "out-of-network" benefits, meaning that they will cover some portion of our fee.  Please contact us if you have questions regarding your insurance coverage. 

If you do not have health insurance, or if your plan has a prohibitively high deductible or no out-of-network benefits, please contact us to discuss our self-pay discount plan.
