Information for New Patients

The first appointment with Dr. Garcia is very important as well as informative.  You are encouraged to bring anyone with you who helps you to make decisions regarding your healthcare and you are encouraged to become an active participant in your healthcare.  During the initial appointment, your medical history will be discussed and based on this information, some tests as well as a physicial exam may be performed.

The most successful initial consultation occurs when your physician knows as much information as possible about your medical history and prior tests or treatments.  Because we want you to receive maximum benefit from your initial consultation with Dr. Garcia, we ask that you address the following important issues before you arrive.

Medical Records

Please obtain a copy of all of your medical records relating to the problem for which you are being seen.  Please have them available for your physician at the time of your first appointment.  You can request that your referring physician's office fax or send your medical records to Center for Women's Surgery.  They will need the completed Release of Information Form in order to send your records to us.  Please ask for all of the following information from your physician or medical care provider:

  • Ultrasound, CT or MRI exams of the pelvis
    • Obtain films or images placed on CD-ROM from the location you had your tests performed
  • Lab tests
    • blood count, hormones or labs related to medical illness
  • Operative reports related to previous pelvic surgery
  • Office notes from other physicians relating to previous evaluation or treatment of the condition for which you are seeing Dr. Garcia

These are forms used by Center for Women's Surgery and are required to be completed at the time of your new patient appointment.  For your convience and to save time at your visit, please download the forms in advance for completion.  Bring the completed forms with you to your first visit.  These forms can be sent to you in the mail or faxed at your request.
